Your first step in engaging Stenum Spine for evaluation and surgery is to complete this evaluation form. This will open your file and insure we have your contact information in our system.
You will then be contacted via e-mail with answers to your specific questions or a request to forward films etc..

Once your evaluation is completed it will be forwarded to you using the e-mail you provided. At this point you will be working with Malte Petersen in Germany.
Malte is head of the International Department and handles all diagnosis and scheduling.
